1. Fill in the registration form and submit it along with a draft of Rs.250/- and other required documents as mentioned below. (Download Registration form)
(Except Birth Certificate and Marriage Certificate, for which 5 photocopies are to be submitted, all other documents are to be submitted in sets of original plus four copies).
1. Birth Certificate or School Leaving Certificate of the adoptive couple.
2. Marriage Certificate
3. Health Certificate of the couple prepared by the family doctor or anyregistered medical practitioner and of biological anddopted children (if any).
4. Gynaecological Report: Infertility should be mentioned, if applicable. This report must include the line of treatment the coupleis undergoing/ has undergone, the period of treatment, and opinion or a reason for childlessness and the chances of the couple having a biological child in future whether nil, remote or fair
5. Photograph: Recent postcard size colour photographs of the adoptive family. (5 copies)
6. Recommendations: Three letters of recommendation from persons who know the family well enough to recommend them as a married couple, as persons and as prospective adoptive parents. Two of these letters should be from parents or immediate relatives of each spouse.
7. Property Statement: A statement on the value of property owned by the family, including other assets like bonds, shares,policies, fixed deposits, etc. and liabilities
8. Employment Certificate: Income certificate from the employer indicating gross and net salary, designation and also the length of service of the husband and wife in case the wife is also employed. In case of a self-employed person, income tax returns or acertificate from a Chartered Accountant giving details of the person?s type of employment and monthly or annual income.
9. Bank Reference: Indicating the current bank balance of the adoptive family.
10. Child care arrangements: If both husband and wife are working, a joint statement expressing what child care arrangements they foresee making when they have a child.
11. Undertaking: From a family member expressing his/her willingness to look after the child in case of any short or long termeventuality. In case of couples who are above 40 years of age, this should be provided by a younger relative.
ADDITIONAL DOCUMENTS, IF APPLICABLE:
- In cases where couples have a biological and / or adopted child / children and if they are old enough, they will be required to express their thoughts on paper to convey their reaction to their parents decision to adopt.
- A copy of adoption decree(s), if the couple have adopted child / children earlier.
- Divorce decree in case the spouse / spouses were earlier divorced.
